2025 Tempe Sister Cities Student Ambassador Selection Information & Application
In order to be considered for Tempe Sister Cities’ 2025 Student Exchange Program, high school juniors / students are required to submit 3 types of forms online:
1. Student Application Form
2. Parent Form
3. Teacher Evaluation Form (from 2 – 3 teachers)
We strongly recommend that applicants and parents complete their respective forms on a word doc on your own personal computer first. Once the forms are completed, come back to this site and cut and paste your answers into the online form.
Following these steps can help you in this process.
Step #1: Identify 2 – 3 teachers that know you well and would be willing to complete an evaluation on your behalf. It’s a good practice to discuss the Student Exchange Program and the evaluation form with the teachers first. Then send the teachers an email where you request their assistance in completing an online evaluation/form for you. The form should take them less than 10 minutes to complete. Provide the teachers with a link to the TSC Student Exchange Program website and a link to the Teacher Evaluation form. Don’t forget to give them the deadline for submitting the evaluation on your behalf. You’ll also want to check-in with your teachers to see that they received your email, and completed the form before the deadline.
Step #2: Download the Student Application and Parent form word docs to your own personal computer. Read through both forms so you know what information you’ll need to gather, like a current photo and a photo of your signature and your parents’ signature.

Step #3: Complete the Student Application and the Parent form (Word docs) on your personal computer. Make sure you save this document so you don’t lose your work. When you are happy with your answers, you are ready for the next step.
Step #4: Complete the Student Application Form and the Parent Form on the TSC website.
Step #5: You’ll receive an email from Google Forms with a copy of your submission. If you don’t receive this email, there may be a problem with your submission. Please contact Joe Forte at 602-926-0236.
